Abstract

Indeks jakosti disfonije (DSI) je objektivni višeparametrijski mjerni instrument koji na temelju najviše frekvencije (F0-High), maksimalnog vremena fonacije (MPT), najnižeg intenziteta (I-Low) i jittera kvantificira promjene glasovne kvalitete. Koristi se kao sastavni dio cjelovite kliničke procjene, a njegov je značaj prepoznat ponajprije u dijagnostici, ali i praćenju pacijenata s poremećajima glasa. Cilj ovog istraživanja bio je odrediti Indeks jakosti disfonije kod osoba s dijagnosticiranim poremećajem glasa, te utvrditi postoje li razlike između ispitanika s obzirom na uzrok i razlikuju li se njihovi rezultati od osoba bez vokalne patologije. Istraživanje je bilo provedeno na 20 osoba (17 žena i 3 muškarca) s dijagnosticiranim benignim promjenama čije su dijagnoze podijeljene u dvije skupine: organski i funkcionalni poremećaji glasa. Rezultati grupe s disfonijom uspoređivani su međusobno, tj. između etioloških grupa kao i s kontrolnom skupinom koju je činilo 11 osoba (8 žena i 3 muškarca). Indeks jakosti disfonije određivan je na temelju snimljenih fonacija iz kojih su potrebni parametri izdvojeni analizom u programu Praat. Dobiveni rezultati obrađeni su u programu SPSS. Rezultati istraživanja pokazali su postojanje statistički značajne razlike na Indeksu jakosti disfonije između skupine ispitanika s poremećajem glasa i ispitanika bez poremećaja glasa. Također, značajne razlike zabilježene su na varijabli maksimalno vrijeme fonacije. Razlike u rezultatima DSI nisu pronađene između funkcionalnih i organskih poremećaja glasa. Indeks jakosti disfonije pokazao se kao valjan mjerni instrument u utvrđivanju poremećaja glasa, tj. odvajanju glasova narušene kvalitete od zdravih glasova. Međutim, u ovom istraživanju nije se pokazao dovoljno osjetljivim za diferenciranje disfoničnih glasova što se može pripisati metodološkim nedostacima

Abstract (english)

The Dysphonia Severity Index (DSI) is an objective multiparameter measure which quantifies vocal quality changes based on highest frequency (F0-High), maximum phonation time (MPT), lowest intensity (I-Low) and jitter. It is used as part of the complete clinical assessment and its value has been recognised primarily in diagnostics, but also in monitoring patients with voice disorders. The aim of this research was to determine the Dysphonia Severity Index in patients with a diagnosis of a voice disorder and to establish whether there are differences between the scores of the participants with regard to the cause and whether their scores differ from those of the participants with no vocal pathology. The research was conducted on a sample of 20 participants (17 female and 3 male) with a diagnosis of benign changes. Their diagnoses have been divided into two categories: organic and functional voice disorders. The scores of patients with a dysphonia were compared with other scores within the group, that is among the etiological groups, as well as with the scores of the control group, which consisted of 11 participants (8 female and 3 male). The Dysphonia Severity Index was determined based on the recorded phonations, from which the parameters needed were abstracted using the Praat software. The scores gathered was processed using the SPSS software. The results of the research have proved the existence of a statistically significant difference in the Dysphonia Severity Index between the group of participants with a voice disorder and the participants without a voice disorder. Furthermore, significant differences were also found with regard to the maximum phonation time parameter. No differences in the scores of DSI have been found between the functional and the organic voice disorders. The Dysphonia Severity Index has proved to be a valid measurement in the assessment of voice disorders, that is in differentiating voices of a reduced quality from normal voices. However, it has not proved in this study to be sensitive enough to differentiate between dysphonic voices, which can be ascribed to methodological issues
